MEDI-CAL APPLICATION & ELIGIBILITY
Medi-Cal (called Medicaid in other states) provides health care coverage to low-income California residents who qualify. In Shasta County, you must first apply for Medi-Cal through the Department of Health & Human Services Agency. Once the application is approved, you will then transition, between 30-60 days, to a membership with Partnership HealthPlan of California (PHC) which is a non-profit organization that contracts with the state to manage all of your health care benefits and claims. Providers will submit their bill to PHC for payment. Some of the services include medical, dental, prescriptions, vision, long term care in skilled nursing facilities, Medicare costs, etc. Medi-Cal and Partnership are often used interchangeably. Stop by in person or call Shasta County Department of Health and Human Services (see below) for an application to be mailed to you. You can also get an application online at C4Yourself.com.
Please note: There are only certain Medi-Cal programs that will not be managed by PHC. Call 1 (877)652-0731 for more information.
